1. Acknowledgment of Risks I acknowledge that participating in tennis activities at Wave Tennis involves inherent risks, including but not limited to, physical injury, equipment malfunction, and environmental hazards. I understand that these risks can arise from my own actions, the actions of others, or the use of club facilities and equipment, including ball machines.

2. Assumption of Responsibility I voluntarily assume all risks associated with my participation in tennis activities at the club. I agree to take full responsibility for my safety, including the use of club facilities, courts, and ball machines. I further acknowledge that I am in good health and physically capable of participating in such activities.

3. Waiver and Release of Liability I hereby waive, release, and discharge Wave Tennis, its owners, employees, coaches, agents, and representatives from any and all claims, liabilities, demands, actions, or causes of action arising out of any injury, loss, or damage that may occur while using the club’s facilities, whether caused by negligence or otherwise.

4. Use of Ball Machines I understand that ball machines require proper handling and can cause injury if not used correctly. I agree to follow all club rules and instructions regarding ball machine use. I release Wave Tennis from any liability arising from misuse or malfunction of the ball machines.

5. Medical Responsibility I understand that Wave Tennis does not provide medical staff or emergency medical support on-site. I acknowledge that I am solely responsible for my own health, safety, and any necessary medical care while using the club’s facilities. In case of an emergency, I understand that I must take appropriate action to seek medical assistance on my own.

 

6. Compliance with Club Policies I agree to follow all club safety policies, rules, and regulations. I understand that failure to comply may result in suspension or termination of my privileges at the club.

 

7. Indemnification I agree to indemnify and hold harmless Wave Tennis from any claims, damages, or costs resulting from my participation in club activities.

 

8. Consent for Minors (If Applicable) If the participant is under 18, a parent or legal guardian must acknowledge and accept all terms of this waiver on behalf of the minor child.

 

Electronic Acknowledgment By checking the box during the online booking process, I confirm that I have read, understood, and voluntarily agree to the terms of this waiver. I acknowledge that this electronic agreement is legally binding and serves as the equivalent of a signed document.
